Get Instant and On-Demand Access to the  POLST and Advance Directives ADiE Registry at No Charge

The COVID-19 crisis reminds us again that providers need instant access to Advance Directives and POLST forms at the point of care.

Providers across the country are facing the challenge of providing medical care to critically ill patients; they do not have the time to stop and request copies of advance directives or POLST forms. Having access to documented end-of-life care wishes ensures that patients receive care that is consistent with the patients values and beliefs. 

At Care Directives we would like to help providers during this national public crisis by providing access to the ADiE (Advance Directive information Exchange) application at NO CHARGE!

Here are some answers to some frequently asked questions:

What is ADiE?

ADiE is a centralized repository for advance care planning documents like advance directives, DPOA- Healthcare, POLST forms and Out of Hospital DNR forms. Forms are submitted to Care Directives by hospitals, nursing homes, hospice agencies and physicians, once reviewed and digitized they are made available to providers at all levels of care across the country. 

Who is eligible for a complimentary ADiE account?

Any provider or healthcare organization providing patient care (COVID-19 and Non-COVID-19) that needs access to patient advance care planning document.

 

How long will providers have access to their complimentary free ADiE account?

Through the cessation of each emergency declaration by State or State and National Health Departments. Provider in all states are eligible even if they serve in a state that has not declared a state of emergency as they may be handling overflow case from surrounding states. 

 

How do I get started? 

Getting started is fast and easy. Simply register online or contact Care Directives support.

How does ADiE work? 

Once a user has successfully registered with Care Directives, he or she can visit the ADiE login page at https://adie.caredirectives.org/#/, from there the user can search the registry and access any POLST form or Advance Directive document that might be on file.
