Dialysis Centers

Bridging the communication gap between  providers and care settings is not easy. Dialysis centers provide care to a large frail population; many of whom reside in long term care facilities. Patients who reside in a skilled nursing facility or assisted living facility go to and from dialysis centers up to three times a week. Sometimes those same patients get transferred to the nearest emergency room due to a sudden decline. Having a paper care directive form follow the patient between care settings is a broken and ineffective system.
